windshield wiper factory five jig for drilling holes needed

Hi does anyone have the jig used to drill the holes for the windshield wiper arms. I built a car for a customer and 85 miles in and he crashes it so putting a new front end on but i gave him everything back when i was finished and he can't find it. I am willing to buy, rent, pay for shipping whatever it takes if i can get one.

Tried to create a label to ship the jig but USPS doesn't like the address. Keeps changing it to 6336. Any thoughts or a different address?

thank you so much for getting the jig to me i will be returning it in the mail tomorrow got the car done and the customer picked it up yesterday it is really awesome when people like you help others out there isn't enough of that in this world i try to whenever i can and it is appreciated when others do it.
