Installing clutch fork w/cable

I just received a pile of bits from Mike Forte; TKO600, flywheel, clutch, bellhousing and his 'ford cable conversion' which seems to have the fork, a big thick plate and a few washers. I understand how to fit the fork, with the correct fitment on the bearing. I have no idea what to do with the plate - I can't find what the holes in it line up with!!

Yes, I know I'll feel dumb when I find out the answers. But I'm not too proud to ask :)

What do I do with the plate, and how does the cable fit into the rod end?:

A quick call to Mike gave me the hint I needed - it wasn't what I had in my head. I pictured the cable outer fixed at both ends and the cable moving. But the cable is fixed one end, and the outer secured the other.

This is very rough mockup of course, fork isn't attached to anything..
