Anybody from texas, that has filled out the Form 130-u for title and registration recently, that can tell me what write under the top section of the form. In reading the DMV Manual I am confused what to use in the year, make and model.
Any help would be great.

Mine is registered as a “Custom Vehicle” which is annual inspection exempt. Year is that of construction, Make is ASVE, and Model is SHE. Body Style is RD. My registration also has a Vehicle Record Notation of Replica:[1965 COBR] which might also go on the form someplace. Hope that helps.
Download the assembled vehicle manual. I also registered mine as a custom vehicle.

Same here.
Had to get the ASE Certification form signed by a Master Technician and get the car weighed on certified scales as well.

On the 130-U, I put:
Year = the year on the MCO,
Make = ASVE,
Body Style = RD
I also put MK4 for the Model
I'm not saying thats the way to go, but I had no issues at all.
I also took the Assembled Vehicle Manual with me to the Title Office and wound up explaining everything to the clerk as she had never done one.
I acted like I knew what I was talking about and she just did what I said (I had no clue if I was right because its just not that clear).
I think the state will just send it back for correction if there is an issue.

I did mine the same as those above, MCO year, ASVE make, and RD body style. Be prepared with everything listed in the manual and you will get a temporary plate. Note though that a few of us lately have gotten a temp plate and then instead of receiving the title, received a letter telling us it was cancelled. My letter asked for more forms, pictures, etc... Once all that was sent in, I finally got my title. The important thing to know is that the local office people don't make the final decision. That is handled centrally.
