CT registration application form R-95 questions

I was looking at the CT R-95 application form, and was wondering what others have filled out for some of the information. I have a 1987 351 windsor , tremec TKO, and a 1997 Lincoln mark8 rear axle IRS

what did you fill in for

Rear Axle ID Number =
Parking brake Type =
Body Style =
