Mass Registration - Donor Paperwork

Looking for examples of the paperwork used for donor components (engine/tranny) that had the correct information to satisfy the MA State Police that the hardware wasn't stolen. I'm buying a used 69 351 block and T5 transmission from a 89 Mustang restoration shop and want to make sure I get the appropriate info on the bill of sale. Below is what I was going to use, comments/suggestions?



• 1969 Mercury 351 Windsor block, casting # C90E-6015-B, date code 8J20.

Partial VIN # 1H510234 (stamped in block), removed from 1969 Mercury

Cougar VIN # 9F91H510234 (17 digits started 1981)

• 1989 Ford T5 Transmission removed from 1989 Ford Mustang VIN #

Receipt showing the basic descriptions of the parts purchased. VIN and additional details a bonus. Also make sure you show you paid sales tax in MA on it.
