installed upper control arms and want to make sure i mounted them right. The manual says use the top mounts, the video ff5 makes shows them on side mount. Using 1994 spindles so assume the top is correct?

Yes, when using the Ford Spindles with the adapter brackets you use the holes on the top of the frame for the upper control arm mounts.
The FF-5 spindles use the other holes.

The video shows using washer behind forward bolt, manual doesn't say for top mount to use washer, do you use the washer for top mount under nut?

After reading the post I second guessed my UCA bolting location. Ran down to the graage and doubled checked!

I think the reason they say not to use a washer on the rear for the FF mounting location is that a washer won't fit in the space. In fact, I couldn't even get a socket on the nut due to the clearance and ended up using a flat screw driver to wedge the nut to allow me to tighten and torque the rear bolt. I'd say if a washer will fit, use one.

Same thing on my car: no room for a washer. I also had to use the flat screw driver trick to get the bolt tightened.
