I have a donor rear end with swapped out 4-lug to 5-lug conversion from Mike Forte.

I have the following ebrake cable parts:
T-Cable (www.cjponyparts.com/parking-brake-cable-assembly-8-long-front-1987-1993/p/PBCF16/) From CJ Pony Parts
M2809A (www.amazon.com/gp/product/B00DQRMMKU/ref=oh_aui_detailpage_o00_s00?ie=UTF8&psc=1) Ford eBrake Cable from Amazon

How do I connect that M2809A brake cable to the 5-lug disk brake setup from Forte's?

On the rear caliper, you're pulling from the wrong direction. The lug on the cable goes in from the other side and pulls the opposite direction than what you've pictured. There should be a mounting point for the fixed portion of the cable, but it's not in the frame of the picture you posted. For the T-Cable, or equalizer as it's often called, hard to tell from the picture you posted. But it's a pretty normal connection to the Mustang handle then it wraps a quarter turn or so around the pulley. You can lose the rubber boot. Does nothing in this installation other than get in the way.

Found it : there is indeed a built-in bracket with a hole for the cable on the caliper assembly in the opposite direction as you indicated edwardb. However, looks like I may have bought the wrong cable as it is too fat to fit inside that cable holder on the caliper. Damn.
